The Community Health Accreditation Program (CHAP) is an independent, non-profit accrediting body specific to community based healthcare services. CHAP serves to validate the delivery of quality community health care through the survey and site visit process. Backus Home Health Care achieved accreditation in June 2009.
The Home health Quality Improvement (HHQI) is a national campaign designed to improve patient outcomes. The Centers for Medicare and Medicaid Services (CMS) has kicked off a national initiative across healthcare settings from acute care to home health care. The goal of this initiative is to improve the care provided to our patients by sharing best practices to reduce preventable hospitalizations and improve medication management.
